Comparison of Recurrence Pattern: Sublobar Resection Versus Lobectomy for Staging I Non-small Cell Lung Cancer

Purpose : Recent studies have suggested that sublobar resection such as segmentectomy or wedge resection may be an acceptable alternative treatment to lobectomy for surgical procedures of early stage non-small cell lung cancer (NSCLC) This study retrospectively compared the recurrence pattern and the operative characteristics of sublobar resection versus lobectomy in patients with pathologic staging I NSCLC.
Methods : We performed a retrospective analysis of 879 medical records of the surgical treatment of pathological stage I NSCLC at our institution between 1 January 2009 and 31 December 2015. We assessed the operative characteristics, recurrence, and survival differences using chi-squared test, Kaplan-Meier approach.
Results : A total of 70 patients underwent sublobar resection including segmentectomy and wedge resection and 809 patients had lobectomy. The recurrence rate for the 70 patients who underwent sublobar resection was 10%, compared with 5.9% in 809 patients with lobectomy. Loco-regional recurrence was found in 4 of the 70 sublobar resections (5.7%) and in 24 of the 809 lobectomies (3.0%). The difference in recurrence-free survival in patients with sublobar resection compared with lobectomy was not significant (p=0.271).
Conclusion : Our results suggested that the patients who underwent sublobar resection may achieve similar recurrence patterns with those of patients who received lobectomy for pathological stage I NSCLC.
