Initial Experience of Sutureless Valve and Rapid Deployment Valve

Purpose : Recent technological developments have led to an alternative minimally invasive option which avoids the placement and tying of sutures, known as “sutureless” or rapid deployment aortic valves. We report an initial experience of sutureless valve and rapid deployment valve.

Methods : From July 2015 to July 2017, 21 patients underwent aortic valve replacement using sutureless valve or rapid deployment valve. Seventeen patients (81%) underwent aortic valve replacement with sutureless valve. Mean age was 76.3±5.9 years. Twelve patients were female. Mean STS score was 8.6±7.5(3.3-25.2). Mean ejection fraction was 57.5±14.0%. Mean pressure gradient was 65.3±6.6.One patient with AR underwent AVR. Seven patients(33%) underwent AVR under minimal invasive technique. Two patient with type I bicuspid aortic valve underwent AVR using rapid deployment valve. Three patients underwent AVR combined with CABG. One patient underwent re-replacement of aortic valve and mitral valve.

Results : One patient with bicuspid aortic valve underwent re-implantation of stented valve due to para-valvular leakage. One patient required implantation of permanent pacemaker. No valve thrombosis, migration, or structural valve deterioration occurred. New York Heart Association class improved at least 1 level in 100%. Total bypass time was 75.7±19.4 minutes and total aortic cross clamping time was 35.5±12.5 minutes. Mean systolic pressure gradient was 11.5±2.6 mm Hg after operation.

Conclusion : Aortic valve replacement using sutureless valve could offer an alternative treatment option. Despite the advantage of sutureless and rapid deployment valve, adequate preoperative assessment may be required to reduce the postoperative complication.
