Negative Pressure Wound Therapy for the Opened Wound following Partial Excision of the Infected Arteriovenous Graft

Purpose : Postoperative wound care for arteriovenous graft infection is time-consuming process to require a great effort of physicians and patients. We recently adopted the negative pressure wound therapy for the management of the opened wound following partial excision of the infected arteriovenous graft.

Methods : From January 2015 through June 2017, 15 patients underwent total or partial graft excision for arteriovenous graft infection. Among them, three patients underwent the negative pressure wound therapy for the opened wound where the affected segment was partially excised. A bypass surgery using a 6-mm straight graft was concomitantly performed to avoid a discontinuation of hemodialysis and the intact portion of the arteriovenous graft was immediately used as a hemodialysis conduit.

Results : The vacuum-assisted closure system was replaced every third day. The opened wound was closed on the seventh postoperative day except the first case that the interval between two procedures was 11 days. In all patients, any evidences suspecting re-infection were not noted during the follow-up period and hemodialysis via the arteriovenous graft was not interrupted.

Conclusion : The negative pressure wound therapy may be a time-saving procedure following the partial graft excision, compared with the traditional treatment that dressings for the opened wound were supposed to be performed twice a day with a delayed wound closure on the 14th postoperative day. A larger study is necessary to evaluate the efficacy of the negative pressure wound therapy for the management of arteriovenous graft infection.
