Various Applications of Intraoperative High Resolution Cone Beam CT in the Thoracic Surgery

Purpose : Since cone beam CT was introduced into the intraoperative angio-system, the hybrid operation which had been applied only to vascular surgery becomes to be possible in the field of thoracic surgery. The purpose of this sturdy is to investigate the feasibility and applicability of intraoperative cone beam CT in hybrid thoracic surgery.
Methods : From Sep 2016 to July 2017, 46 patients underwent hybrid operation or intervention using intraoperative cone beam CT (Dyna CT, Artis Zeego Q, Simens). We retrospectively reviewed clinical data.
Results : We performed intraoperative targeting of pulmonary nodule in 26 patients. In the initial 11 patients, the needle-guided targeting were performed and we changed the targeting technique to the clip-guided targeting since Feb 2017. After localization, all of them were underwent wedge resection via single port incision. Eighteen patients were diagnosed as lung cancer. Six of them underwent SITS lobectomy and 12 underwent segmentectomy. Of the remaining 8 patients who underwent wedge resection, 6 were benign and 2 were metastatic. There were no mortality and morbidity. Additionally, we performed other types of hybrid operations in 20 patients. We performed CT-guided empyema drainage in 11 patients, the RF ablation in 4 patients, the SVC intervention in 3 patients and other interventions in 2 patients (Table 1).
Conclusion : The Intraoperative cone beam CT is usefully applicable in the field of targeting the solitary pulmonary nodule and various CT guided hybrid thoracic surgery and intervention. It will be applicable to more various thoracic surgery and helpful to overcome the limit of high difficult operation.
