Surgical Resection of Huge Cervical Invasive Thymoma after Induction Chemotherapy

Purpose : Although invasive thymoma commonly infiltrates neighboring mediastinal structures, its extension into the cervical portion are rare. Complete resection is the key treatment of thymoma. Induction treatment may improve the ability to achieve a complete resection.

Methods : We report a case of surgical resection of huge cervical invasive thymoma after induction chemotherapy. The tumor infiltrated into left thyroid, upper trachea, both carotid and left subclavian artery. The size of whole tumor is about 9cm. Fine needle biopsy (FNB) was performed on the thumor and the result was confirmed as thymoma. We decided to perform induction chemotherapy first after considering that there is some difficulty in surgical resection.

Results : There was no significant change in the evaluation after two cycles of chemotherapy, and we decided to surgically resect it. After partial sternotomy, mass was connected from anterior mediastinum to neck and thyroid. Mediastinum mass was well separated from surrounding tissues, but mass around neck was highly adherent and invasion to thyroid was suspected. So we performed left thyroid lobectomy under GS co-operation.

Conclusion : The tumor was well resected without major problems. Postoperative pathologic examination revealed both mediastinum and thyroid as thymoma and margin was negative.
