Analysis of the Relationship between Recurrence after the Operation of Primary Spontaneous Pneumothorax and the Number of used Stapler

Purpose : Primary spontaneous pneumothorax is a disease with high recurrence rate and requires surgical treatment at the time of recurrence. In general, the wedge resection has performed, and many methods have been implemented to reinforce to prevent bullae formation at the wedge resection site. The range of the weak point changes depending on the number of staples used during the wedge resection, so we analyzed the relationship between the number of staplers used and the rate of recurrence after surgery.
Methods : We analyzed 614 patients under 40 years of age who underwent wedge resection from January 2011 to December 2015 at our hospital with primary spontaneous pneumothorax. Other types of pneumothorax were excluded except for the primary spontaneous pneumothorax and patients who did not use a surgical stapler were also excluded.
Results : Total 614 patients were analyzed, 572 (90.4%) were males and 61 (9.1%) were females. The mean age was 19.9 ± 5.6 years, and 94 patients (14.8%) had postoperative recurrence. The mean number of staplers used was 1.79 ± 0.98 in recurrence group and 1.71 ± 0.87 in no-recurrence group, did not show a statistically significant difference.
Conclusion : After Wedge resection, various methods such as mechanical pleurodesis, absorbable mesh apply, and glue apply are carried out for reinforcement procedure, and difference in recurrence rate between each method was not significant. The possibility of new bullae occurring at the site using stapler could be seen as not affecting recurrence rate after surgery when performing an appropriate additional reinforcement procedure.
