Cervical Drain Amylase Level as a Predictor of Anastomotic Leakage after Esophagectomy

Purpose : Anastomotic leakage after esophageal cancer surgery is a serious complication. The purpose of this study is to evaluate the possibility of anastomotic leakage by repeatedly measuring amylase through the drainage tube inserted on the anastomotic site.
Methods : We analyzed 64 patients who underwent esophagectomy and reconstruction with esophageal cancer at our hospital from April 2014 to July 2017. All patients underwent cervical anastomosis and a drainage catheter was inserted around the anastomotic site. From the day of surgery or the 1st postoperative day, the drainage amylase level was measured repeatedly daily until oral feeding or confirmed the anastomotic leakage.
Results : Total of 64 patients, 57 males (87.7%), 7 females (10.8%), and the average age was 64.1 ± 9.0 years old. Anastomotic leakage occurred in 8 cases (12.3%). The drainage amylase level was higher in leakage group than in normal group (p = 0.043) (Fig 1). Comparing the leakage group and the normal group, the drainage amylase level did not show a significant difference until the 3rd postoperative day (p=0.766, 0.861, 0.206, respectively). But from the 4th postoperative day, drainage amylase level was significantly higher in leakage group (p=0.032, 0.001, 0.000, respectively).
Conclusion : Drainage amylase levels may be predictive of anastomotic leakage after esophageal cancer surgery. Especially the elevation of drainage amylase level after the 4th postoperative day may be regarded as an indicator of anastomotic leakage. Therefore, careful observation, active diagnosis, and rapid treatment are required for patients with elevated drainage amylase at that time.
