Impact of Resection Margin Length on the Locoregional Recurrence after Pulmonary Metastasectomy of Colorectal Cancer

Purpose : The influence of tumor size, depth on preoperative CT image, and resection margin length on locoregional recurrence after pulmonary metastasectomy for colorectal cancer is unclear. In this study, we analyzed the impact of length of resection margin on the locoregional recurrence-free survival.
Methods : All patients undergoing pulmonary wedge resection for single colorectal cancer pulmonary metastasis from 2007 to 2016 were analyzed. Patients were divided into two subgroups based on the pulmonary resection margin: 0.1 – 0.5 cm (group 1), >0.5 cm (group 2).
Results : Among 72 patients, a total of 48 patients were included in this study after exclusion of patients with incomplete resection, incomplete data, and follow up loss. 17 (35.4%) patients had locoregional recurrence (group 1: 12/33(36.4%), group 2: 5/10(50%), p=0.553). Median follow up period was 34 months (range 8 – 101 months), and median locoregional recurrence-free survival was 25.50 months (range 1 – 97 months). Median tumor size and depth on preoperative CT were 1.1 cm and 1.6 cm, respectively. Median length of resection margin was 0.5 cm. No difference was noted in locoregional recurrence-free survival between the two subgroups (median: 28 months (group 1), 25 months (group 2) (p=0.978)). No correlation was noted among the tumor size, depth, and the length of resection margin.
Conclusion : Length of resection margin in pulmonary metastasectomy does not seem to affect significantly on the locoregional recurrence if complete resection is accomplished. Also, preoperative tumor size and depth on CT imaging does not seem to affect on the length of resection margin during metastasectomy.
