The Feasibility of One Stage Combined Thoracic and Abdominal Surgery using Minimal Invasive Thoracic Approach

Purpose : In case that intrathoracic masses are found in a patient scheduled for abdominal surgery, the one stage surgery is not preferred because the high risk of complication. For one stage surgery, as less invasive as possible thoracic approach are necessary.
Methods : From April 2011 to July 2017, 41 patients underwent one stage thoracic and abdominal surgery. . We retrospectively evaluated the feasibility of one stage surgery
Results : The table 1 summerized the one stage combined thoracic and abdominal surgery. The type of thoracic surgery was subxiphoid approach 19 cases (46%), single- incision thoracoscopic surgery (SITS) 11 cases (26%), multi-port VATS 7 cases (17%), open thoracotomy 2 cases (4%), sternotomy 2 cases (4%). The types of thoracic surgery were 15 cases (36%) of wedge resection, 9 cases (21%) of multiple wedge resection, 5 cases (12%) of segmentectomy, 6 cases (14%) of mediastinal mass excision, 3 cases of lymph node excision (7%) and 4 cases (9%) of others. There was one abdominal surgery relate mortality.The median thoracic and total operation time was 90min (range, 13-430 min) and 340min (range, 135-595min) respectively. The median hospital stay was 8.5 days (range, 3-42 days) and. Three patients had prolonged hospitalization by thoracic surgery
Conclusion : The risk of one stage thoracic and abdominal surgery was not high .The active one stage operation is recommended if intrathoracic masses are incidentally found in a patient scheduled for abdominal surgery.
