Long-term Results of Tricuspid Annuloplasty using the MC3 Ring for Functional Tricuspid Regurgitation

Purpose : The aim of this study was to evaluate the long-term results of tricuspid annuloplasty using the MC3 ring for functional tricuspid regurgitation (TR).
Methods : Between December 2004 and December 2014, 249 patients underwent tricuspid ring annuloplasty using the MC3 ring for functional TR (FTR). The etiology of FTR were left heart valvular disease (n = 201), left to right shunt (n = 36), mixed lesion (n = 3). The TR was graded from 0 to 4; 0=no or trace, 1=mild, 2=moderate, 3=moderately severe, and 4=severe. The clinical follow-up duration was 84.4 ± 37.1months.
Results : Operative mortality occurred in 7 patients (2.8%). Both TR grade and systolic pulmonary artery pressure (sPAP) significantly decreased compared to its preoperative values in predischarge echocardiography and the values sustained during follow-up (from 1.73±0.98 to 0.68±0.61, p<0.001, and from 48.4±14.9 to 37.7±10.3mmHg, p<0.001, respectively). There was no ring-related complications both postoperative and follow-up periods except for permanent pacemaker implantation in 2 patients. Freedom rate from TR ≥moderate and tricuspid valve related events at 10 years were 92.1% and 91.4%, respectively. The TV annular size was a risk factor for recurrent of TR ≥moderate (p = 0.001) and the minimal P value approach demonstrated that TV annular size of 42mm was a cut-off value predicting the recurrent of TR ≥moderate.
Conclusion : Tricuspid annuloplasty using the MC3 ring for FTR provides satisfactory early results and that results remains stable at long-term follow-up. The preoperative TV annular size was associated recurrence of TR ≥moderate.
