Solo Surgery for VATS Thymectomy using Thoracoscope-holding Robotic Device

Purpose : Recently, robotic surgery has been introduced for the treatment of thymic epithelial tumors (TET) with its technical advantages for meticulous mediastinal dissection. However, robotic console system has not yet been widely adopted that most thoracic surgeons performs VATS for mediastinal tumors. With a concept of hybrid technique, we applied thoracoscope-holding robotic device, facilitating solo-surgery for VATS thymectomy for TET.
Methods : A 27 year old male patient was diagnosed with Myasthenia gravis. He was subscribed acetylchonilinesterase inhibitors, but weakness symptoms were not improved. Chest computed tomography scans showed about 3.4 x 2.6 x 4.9 cm sized, anterior mediastinual tumor, suggesting TET. We planned VATS thymectomy under navigation of thoracoscope-holding, which enabled surgeon-controlled visualization without an assistant.
Results : Duration of the operation was 165 minutes, and the estimated blood loss was 20 ml. There were no intraoperative events during whole VATS procedures without an assistant. The pathology results revealed 17.0 x 6.0 x 1.5 cm sized thymic tissues, containing 5.5 x 4.0 x 2.8 cm sized, type B1 TET. The postoperative course was uneventful that the chest tube was removed on postoperative day 1. The patient satisfied with the improved weakness symptoms with cosmetic wounds during follow up.
Conclusion : Our hybrid VATS technique using a robotic device demonstrated the possibility of solo surgery in performing VATS mediastinal tumor resection. This method is thought to be a feasible modification of VATS, having a technical advantages over the Robotic console surgery.
