Polyglycolic Acid (PGA) Sheet with Fibrin Glue Technique in Single Port VATS for Primary Spontaneous Pneumothorax

Purpose : Coverage of polyglycolic acid (PGA) sheet with fibrin glue has been applied to reinforce the staple line with prevention of recurrence. We aimed to evaluate the outcomes of this method without pleural abrasion in single port VATS for PSP.
Methods : Between July 2012 and May 2017, consecutive 54 patients (5 bilateral) underwent single port VATS for PSP using a combination of PGA sheet and fibrin glue technique. We compared PGA sheet coverage first with an additional glue applying group (Group. A), and glue first with a PGA sheet coverage group (Group B).
Results : There were total 59 cases (43 males) with a median age of 18.0 years (12-50 years). Mean operation time was 41.6±9.5 minutes and mean length of the incision was 2.1±0.3 cm. Median length of hospital stay was 4 days (2 to 8 days) with chest tube keeping of 2 days (1 to 6 days). Recurrences were developed in 3/59 cases (5.0 %) during 21.5 months (2.5 to 61.4 months). There was no difference in operation time [41.8±10.1 (Group A) vs. 41.3±8.8 minutes (Group B), p=0.821]. There was no difference in length of hospital stay (4.1±1.36 vs. 3.8±1.1 days, p=0.411) and in chest tube indwelling time (2.6±0.9 vs. 2.2±0.7 days, p=0.078). Recurrence rates revealed no significant difference [2/36 (5.5 %) vs. 1 /23 (4.3 %), p=0.091].
Conclusion : The PGA sheet with fibrin glue technique demonstrated acceptable results regardless of application technique. This simple technique without pleural abrasion is an effective combination to prevent recurrence in single port VATS for PSP.
