2818 Days of Transformation Period, from Pure Ground Glass Nodule to Part Solid Nodule in Combined Breast and Thyroid Cancer Patient.

Purpose : Pure ground glass nodule (GGN) is frequently encountered in clinical setting. Pure GGN could be a benign inflammatory nodule. But, it could be a pre-cancerous lesion or early stage lung cancer. If a pure GGN is tumor, it has relatively long doubling time, more than 800 days. Recently, we experienced a case of extremely long transformation period from pure GGN to part solid GGN. Here we report our case with related literatures.

Methods : Retrospective chart review

Results : 51 years female patient underwent total thyroidectomy for thyroid cancer and wide excision followed by chemo and radiation therapy for breast cancer. Simultaneously, two lung nodules were found. RLL and LLL wedge resections were done after gold pin localization. Lesions were all benign. After 20 months, multiple small nodules were newly noted at bilateral lungs. Surgical biopsy was performed at LLL and the result was AAH. Then about 8 years later from initial CT finding, RLL lesion had increased, significantly and also it showed solid component. RLL wedge resection was done and the result was MIA. In pathology, gold pin was found, which was possibly related to about 8 years ago procedure.

Conclusion : In our case, pure GGO took 2818 days to part solid GGN. It was confirmed by the specimen, which was incubating the gold pin. Because the patient was treated with breast cancer chemo and radiation therapy, on the other hand, these treatments might have had some effect on lung lesions, whether it is oncogenic or the contrary. Further study is required.
