Ten-years Survival Comparison between Surgical Off-pump Coronary Revascularization and PCI

Purpose : Surgical and interventional coronary revascularizations are competing for multi-vessel and left main coronary artery in the current era. Our current study was to compare the long-term survival between off-pump coronary artery bypass grafting (OPCAB) and percutaneous coronary intervention (PCI) during first generation drug-eluting stent era (2003-2008).

Methods : 1416 propensity score matched patients with three-vessel and/or left main coronary artery disease were enrolled for analysis. All patients received PCI with 1st generation DES (n=709) or OPCAB (n=707) as their initial treatment. Life-saving surgery or primary PCI procedures were not included. The primary end-point was all-cause mortality and the second end-point was cardiovascular death. Mean follow-up duration was 112 months (0-169). Survival analysis was based on the database from Korean Statistical Office.

Results : Follow-up was completed in all patients (follow-up rate 100%). 239 deaths occurred in PCI group and 177 deaths in OPCAB group (33.7% vs 25.0%, p<0.001). Cardiovascular deaths occurred 107 in PCI group and 70 in OPCAB group (15.1% vs 9.9%, p<0.001). 10-year survival rate was 66.7±1.8% in PCI group and 76.4±1.6 % in OPCAB group (p<0.001). 10-year cardiovascular survival rate was 83.2±1.5% in PCI group and 89.9±1.2% in OPCAB group (p=0.001).

Conclusion : OPCAB showed 9.7% survival advantage in overall mortality and 6.7% cardiovascular survival advantage after 10 years follow-up in three-vessel and/or left main coronary artery disease patients compared to PCI with first-generation DES.
