Comparison between Sublobar and Lobar Resection in Octogeneraians with Pathologic Stage I Non-small Cell Lung Cancer

Purpose : Sublobar resection may be a potential alternative to standard lobectomy for elderly patients because of reduced operative risk and better preservation of pulmonary function. This study is to compare the overall survival between sublobar and lobar resection in octogenarians with stage I non-small cell lung cancer(NSCLC).

Methods : From 2003-2016, 77 octogenerians who underwent curative resection for stage I NSCLC were enrolled. The clinical data were collected retrospectively,comobidity, smoking status, COPD, pulmonary function, number of resected LN,complication rate and hospital stay. Sublobar resection was planned if tumour size is small or safety margin is sufficient. Overall survival(OS) and Recurrence free survival(RFS)were analyzed by the Kaplan-Meier method and log-rank test. Cox proportional hazards regression analyses were performed to identify prognositic factors.

Results : There were 53 with lobar resection and 24 patients with sublobar resection. There was no difference in sex, smoking,performance and comorbidities except presence of COPD between two groups.The smaller number of dissected LN,shorter hospital stay and lower complications rate(13%vs26%) was found in sublobar resection.The median follow up duration was 24 month.The OS (P=0.354)and RFS rate(p=0.623) were not statistical difference between two groups(Fig.1,2).In mulivariate analysis,COPD and family history were shown as a risk factor for RFS whereas tumour size and vascular invasion were risk factor for OS.

Conclusion : Sublobar resection was an alternative treatment for stage I NSCLC in octogenarians.Sublobar resection showed similar results of OS and RFS to lobar resection.
