Risk Factor Analysis for Acute Subdural Hemorrhage after Aortic Surgery

Purpose : Acute subdural hemorrhage following open-heart surgery is known to be uncommon, but it may cause fatal complications perioperatively. We investigated the incidence and risk factors of acute subdural hemorrhage following aortic surgery.

Methods : From June 2016 to July 2017, there were 34 patients (18 men, 16 women; mean age, 66.9±12.5 years) who had thoracic aortic repair with open distal anastomosis and required selective antegrade cerebral perfusion. All patients performed brain computed tomography within 24 hours from aortic surgery with or without neurologic symptoms. The patients were divided into two groups: a subdural hemorrhage group and non-subdural hemorrhage group. The variables were compared between the two groups.

Results : Ten (29.4%) patients had a subdural hemorrhage on brain computed tomography postoperatively. The presence of neurologic symptoms did not predict the occurrence of acute subdural hemorrhage (p=0.328). The incidence of acute subdural hemorrhage tended to increase with younger age, women, history of hypertension, longer operation time, diagnosis as aortic dissection. Multivariate analysis revealed that a significant risk factor for acute subdural hemorrhage after aortic surgery was the age (odds ratio =0.8, 95% confidence interval: 0.68-0.98, p=0.029).

Conclusion : The incidence of acute subdural hemorrhage occurs more often than we have thought and it does not always cause neurologic symptoms. Therefore, routine brain computed tomography after aortic surgery, particularly in young ages, is helpful in early detecting acute subdural hemorrhage.
