Case Series: ECMO during MERS-CoV Outbreak

Purpose : Outbreak of MERS-CoV in South Korea brought a total of 186 confirmed patients. The outbreak hit our center very hard as 91 confirmed cases were originated from this center.
Methods : We respectively reviewed patients who had ECMO after MERS infection from May 20, 2015 to July 13, 2015. Although there were four ECMO-MERS patients, one patients refused to use his medical record for research purpose.
Results : A 73-year-old man visited ER with fever and contacted patient 14, the super-spreader. He was discharged from ER and came back five days later with symptoms of fever and cough which was confirmed as MERS-CoV infection. He was transferred to our center. VV ECMO was inserted and maintained for two months. He is still admitted for management of interstitial fibrosis and heart failure.
An 80-year-old woman was diagnosed with both PCA territory infarct was transferred to our center. Fever was checked on Jun 1 and MERS-CoV was confirmed. Pneumonia was progressed and intubation, VV ECMO insertion were done sequentially. Two weeks after VV ECMO insertion, she was died of multi-organ failure.
A 32-year-old man visited ER for fever. MERS-CoV test was positive and was admitted for further work-up and anti-viral therapy. Pneumonia due to MERS-CoV progressed and intubation and subsequent VV ECMO insertion were done. ECMO was weaned and removed on 6th day of ECMO.
Conclusion : Three cases of ECMO during MERS-CoV outbreak at our center resulted in one death, two successful weaning and one successful discharge. Outcome for ECMO for MERS-CoV infection seems to be good.
