Extracorporeal Membrane Oxygenation Support in Organ Transplant Donors

Purpose : The aim of this study was to investigate the effects and clinical outcomes of our experiences with extracorporeal membrane oxygenation in supporting donors before organ transplantation.

Methods : We reviewed 9 brain-dead organ donors (4 men and 5 women, median age 35.0 years; range 21-63 years) from June 2012 to April 2017. Characteristic of donors and early outcomes of transplanted grafts were analyzed.

Results : 5 patients had been supported by ECMO before the brain-death determination procedure and ECMO support had been initiated during brain determination procedure in others, because of deterioration of hepatonephric dysfunction. Median duration of ECMO support was 47.0 hours (range 6-136 hours) and median vasopressor score was 80.0 (range 0-200). Peak values of ALT, total bilirubin and creatinine were 2700 U/L, 9.6 mg/dL and 5.86 mg/dL. 18 kidneys, 7 livers and one heart and pancreas were harvested from donors. No organ was abandoned and all organs were successfully transplanted, although one recipient exhibited acute renal failure required temporary hemodialysis. All recipients could be discharged after recovery.

Conclusion : ECMO can provide effective cardiopulmonary support to transplant donors, improve organ function and provide sufficient time for safe organ donation procedure
