Clinical Correlation between Recurrence Rate of Primary Spontaneous Pneumothorax and Presence of Bullae in Chest Computed Tomography in the First Episode of Pneumothorax.

Purpose : The initial treatment strategy for the first episode of primary spontaneous pneumothorax (PSP) patients with bullae shown in chest computed tomography (CT) is currently controversial because the difference in recurrence rate with or without bullae in chest CT is not clearly established. Therefore, it is hard to evaluate risk-benefit of pleural drainage versus surgical treatment. The aim of this study is to identify the recurrence rate of PSP who have bullae or bleb in chest CT.

Methods : From January 2015 to April 2017, 146 patients with first attack PSP were enrolled. All patients were reviewed retrospectively and followed-up for recurrence in our outpatient clinic for an average of 17.4 months (range, 3 to 30 months). Only ipsilateral recurrences were counted as recurrence.

Results : We analyzed 146 patients. 90 patients of them had bullae in chest CT and 56 of them did not. Recurrence rate of bullae (+) group was 58.9% and non-bullae group was 19.6%. The recurrence was highly correlated with the presence of the bullae and it was statistically significant. (p<0.01) We also analyzed the effects of size and number of bullae on recurrence of PSP with logistic regression analysis. Risk of recurrence increased by 1.09 times with each 1mm increase of bleb size and by 5.86 times with number of bullae.

Conclusion : The presence of bullae in chest CT can be a strong indicator of recurrence of PSP. Therefore, surgical intervention in the first episode of PSP with bullae in chest CT is acceptable for prevention of recurrence of PSP.
