Non-intubated VATS Wedge Resection for Pneumothorax

Purpose : For thoracic surgery, intubation under general anesthesia is conventionally used. However, intubation is invasive and may be technically difficult with unusual anatomy. We aimed to evaluate difference of non-intubated anesthesia using Monitored anesthesia care (MAC), with intubated anesthesia using double lumen, single lumen endotracheal tube, and laryngeal mask airway (LMA), in VATS wedge resection for pneumothorax.
Methods : 138 patients who underwent VATS wedge resection for pneumothorax from September 2015 to July 2017 were retrospectively evaluated; 3 of them with traumatic pneumothorax were excluded. There were 34 cases (25%) of double lumen endotracheal tube insertion; 43 (43%) of single lumen; 28 (21%) of LMA; and 30 (22%) of MAC.
Results : The induction time was significantly shorter for MAC group (p < 0.05), compared to the other 3 intubated-anesthesia groups (21.4±7.1, 16.4±4.9, 16±5.8, and 5.6±3.2 min for double, single lumen, LMA, and MAC, respectively). The operation time (50.2±18.9, 51.4±22.6, 49.4±11.3, and 32.6±7.7 min), and the awakening time (15.5±4.7 min, 13.6±4.9 min, 14.2±5.6 min, and 6.9±2.4 min) were also significantly shorter for the MAC group (p < 0.05). There was no significant difference (p > 0.05) in the duration of chest tube drainage (2.2±1.6, 2.1±1.1, 2.1±1.0, and 1.6±1.0 days) and hospital stay after operation (3.2±1.7, 3.2±1.3, 3.1±1.0, and 2.7±1.0 days).
Conclusion : MAC, compared to intubated anesthesia, shortened induction, operation, and awakening time in VATS wedge resection for pneumothorax; there was no significant difference in terms of the duration of chest tube drainage, and hospital stay after operation.
