How to Overcome the Gap in Perception between the Ideal Implementation of Trans-catheter Aortic Valve Replacement and the Reality? -Based on the Experience of Negotiations for Establishing a Guideline for Monitoring TAVR in Korea-

Purpose : The purpose of the study is to share our experience regarding the process for establishing guidelines and monitoring protocols of TAVR for treating patients in Korea in the hopes that it may be helpful to others undergoing a similar process in their own countries.

Methods : The Korean guidelines for TAVR was established in June 1, 2015 in through a tri-party agreement involving the Dept. of health & welfare, Korean society of thoracic and cardiovascular surgery and the cardiology society. Both professional societies and the Korean government have agreed to monitor the guidelines transparently and exchange opinions regarding amendments or continuation of its contents after 3 years monitoring.

Results : The monitoring meetings were not held strictly as agreed, occurring only twice during the past 2 years (2015.6 - 2017.5). In addition, the meetings were made difficult by insufficient and lax monitoring data. During the meetings, measures to improve the monitoring process were discussed and accordingly, an agreement was made to continue the monitoring process with the aims of completing the data accumulation by 2018.

Conclusion : Compliance to guidelines is critical for assessing the efficacy and safety of treating severe aortic stenosis deemed at high or inoperable risk with TAVR. Moreover, it is essential that the TAVR monitoring process be properly conducted for accurate evaluation. Any country planning to introduce TAVR may encounter difficulties with regards to the optimal initiation strategy and the subsequent monitoring. Nevertheless, continued efforts should be made to persuade the government and the counter-part societies to enable optimal TAVR application.
