Comparison of Japanese Staging System and AJCC 7th, 8th Staging System in Esophageal Squamous Cell Carcinoma Patients

Purpose : Even though American Joint Committee on Cancer (AJCC) staging system has been world-widely used in esophageal squamous cell carcinoma (ESCC), the Japanese staging system also has been proposed by Japanese surgeons. Two systems have different lymph node map, N staging and staging grouping. This retrospective study was performed to compare the predictive ability for the survival of these two staging systems.

Methods : We retrospectively reviewed 146 patients who received the esophagectomy and mediastinal lymphadenectomy in ESCC from 2006 to July 2015. The patients were staged by AJCC 7th, 8th and Japanese staging system. The concordance index (c-index) were compared.

Results : The mean age was 63.21±8.06 years and male patients were 134(91.8%). Median follow-up time was 46.68 (6.27~134.40) months. All patients received the R0 resection. The recurrences were developed in 31(21.2%) patients. Both AJCC N staging and Japanese N staging predicted the disease-free survival (DFS) well (Log-rank test, p<0.001). AJCC 7th, 8th and Japanese staging grouping also predicted DFS well (p<0.001). The c-index of AJCC N staging and Japanese N staging were 0.74 (95% CI 0.64~0.85) and 0.72 (95% CI 0.62~0.82), respectively (p=0.12). The c-index of AJCC 7th, 8th staging grouping and Japanese staging grouping were 0.78 (95% CI, 0.68~0.88), 0.77 (0.068~0.87) and 0.82 (95% CI 0.73~0.90) respectively. The c-index of Japanese staging grouping was slightly higher than AJCC, but was not statistically significant.

Conclusion : Both Japanese and AJCC staging and staging grouping in ESCC showed similar predictive power in DFS. Both system can be applied in cancer clinical situations.
