Feasibility and Safety of Total Mesoesophageal Dissection in Esophageal Cancer

Purpose : The concept of total mesoesophageal (TME) dissection is en bloc dissection of all soft tissue such as thoracic duct and lymph nodes along with esophagus. This retrospective study was performed to investigate the feasibility and safety of TME dissection in esophageal cancer.
Methods : We retrospectively reviewed 238 patients who received the esophagectomy with total mediastinal lymphadenectomy from 2012 to June 2017.
Results : The mean age was 63.27±7.53 years and male patients were 216(90.8%). Seventy-two (30.3%) patients (TME group) received the TME dissection with total lymphadenectomy whereas 166 (69.7%) patients did not receive the TME dissection (non-TME group). The basic characteristics of non-TME and TME group were comparable in terms of age, sex, location of lesion, T stage and N stage. The operative outcomes such as operation time (423.35 ± 86.94 vs. 437.72 ± 83.55, p=0.309), blood loss (187.25 ± 250.08 mL vs. 134.21 ± 148.72 mL, p=0.107), duration of ICU stay (4.41 ± 12.01 vs. 4.32 ± 7.64, p=0.956), postoperative complications (52.4% vs. 50.0%, p=0.778) and operative mortality (7.8% vs. 1.4%, p=0.07) were also comparable. The numbers of total dissected lymph nodes (68.50 ± 28.31 vs. 54.01 ± 24.36, p<0.001) and mediastinal lymph nodes (26.67 ± 18.01 vs. 21.64 ± 14.30, p=0.026) were significantly higher in TME group.
Conclusion : The TME dissection for esophageal cancer was a feasible and safe procedure, and it resulted in higher numbers of dissected lymph nodes in mediastinum. The potential survival benefits have to be studied in the further study with long-term follow-up.
