Experience of Extracorporeal Right Ventricular Assist Device with or without Oxygenator: Case Series

Purpose : RVAD implantation is indicated for refractory right heart failure in various clinical situations

Methods : We respectively reviewed patients who had extracorporeal RVAD implantation at out center. From June 2016 to June 2017, there were 5 patients

Results : A 48-year-old woman was admitted due to chemotherapy induced CMP. After4 weeks of ECMO, LVAD was implanted. She developed ventricular tachycardia and cardiac arrest and emergent RVAD implantation was done. After 10 days, RVAD was successfully removed and the patient was discharged
A 47-year-old man had LVAD implantation for alcohol induced CMP. Few hours after surgery, he developed severe RV failure and RVAD was implanted. RVAD was successfully removed after a weak and he was discharged
A 80-year-old woman was admitted due to repeated ADHF and implanted with LVAD and RVAD because RV dysfunction presented during weaning of CPB. After a week later, RVAD was removed and she is still in the hospital for ventilator_dependency and renal failure
A 49-year-old man was admitted due to ARDS. Although VV ECMO was inserted, he continued to have hypoxemia and RV failure. VV ECMO was converted to extracorporeal oxy-RVAD and RV function was improved. After 2 months, he had lung transplantation and RVAD was removed
A 26-year-old man was admitted with pneumonia and ARDS. VV ECMO was initiated but he developed severe RV failure. Oxy-RVAD was implanted and successfully removed after 10 days

Conclusion : Outcome of extracorporeal RVAD was good. It has a few merits ; keeping pulsatility, no Harlequin syndrome, and less thromboembolic complications, compared with VA ECMO
