The Efficacy and Safety of CT Guided Localization with Hook Wire Prior to VATS

Purpose : The VATS resection of small and deep located pulmonary nodules is
challenging. The aim of this study was to evaluate the efficacy and safety of CT guided localization with hook wire prior to VATS.
Methods : We investigated patients who CT guided localization with hook wire prior to VATS was undertaken from May 2007 to Dec. 2016 due to newly present or growing pulmonary nodules.
Results : A total of 113 pulmonary nodules were located and resected in 113 patients. The median nodule diameter was 10 mm(range 3-30 mm) and the median nodule distance to pleural surface was 18 mm(range 0-30 mm). There were 28 nodules(24.78%) which the distance to pleural surface was more than 25 mm. Wire dislodgement occurred in 5 patients (4.42%). Air embolism during the CT- guided localization occurred in 1 patient (0.88%). Minimal pneumothorax and mild intra-parenchymal hemorrhage during the CT- guided localization occurred in 32 patients(28.32%) and 9 patients(7.96%) respectively. Complications of VATS consisted of 1 pulmonary thromboembolism (0.88%), 16 air leakage (14.16%). The 30 Day mortality was 1(0.9%). Conversion to thoracotomy occurred in 2 (1.77%) patients. Median CT-localization time was 10 min (range 3-40). The pathologic diagnosis consisted of 42 non-small cell lung cancer(37.17%), 2 malignant lymphoma(1.77%), 53 metastatic cancer(46.90%), 16 benign lesion(14.16%).
Conclusion : The CT-guided localization with hook wire is a very efficient and safe procedure prior to VATS for pulmonary nodules and for deep located pulmonary nodules.
