Spontaneous Intercostal Artery Bleeding

Purpose : The most common causes of intercostal artery bleeding is trauma and thoracic procedure. However, there is a very rare occurrence of spontaneous intercostal artery bleeding. We report a successful treatment case of spontaneous intercostal artery bleeding.
Methods : A 60-year-old female patient with dyspnea and dizziness on sudden onset came to our hospital. Massive Left hemothorax suspected of the fifth intercostal artery bleedinge was found in simple x-ray & chest CT(contrast). She denied trauma history and anti-coagulation medication. Emergency operation for bleeding control and hematoma evacuation was performed through VATS approach. Bleeding focus was confirmed and controlled by electrocautery and tachocomb®.
Results : The chest tube was removed on the POD 2 after the simple x-ray and blood test showed no further bleeding and the patient was discharged on POD 3 without any other complication.
Conclusion : The Spontaneous intercostal artery bleeding is a serious condition that can cause a fetal situation if not diagnosed and treated quickly. Intercostal artery bleeding without trauma or thoracic procedure is extremely rare so we report our treatment experience.
