Actual Measurement of Pleural Pressure with High-resolution Pleural Manometer : Preliminary Analysis

Purpose : The negative pressure of pleural cavity is critical to maintain expansion of lungs during respiration. However, it is not well reported the actual value of pleural pressure especially after various type of pulmonary resection. The aim of this study is to measure the pleural pressure directly and to compare the results according to the extent of pulmonary resection

Methods : We manufactured the high-resolution pleural manometer with which we can measure pleural pressure directly through chest tube. We have measured the pleural pressure of the patients who underwent pulmonary resection after air leakage stopped. The pleural pressure of normal spontaneous quiet breathing, forced breathing and coughing was measured.

Results : 7 patients with normal lung underwent lobectomy (lobectomy group), 6 patient with normal lung underwent wedge resection (wedge resection group), and 5 patients with ILD underwent wedge resection (ILD group). The average value of pleural pressure at end inspiration, at end expiration, and the mean pleural pressure were -27.1cm H20, -4.1cm H20 and -12.6cm H20 respectively in lobectomy group. In the same order, -17.1cm H20, -4.6cm H20, -8.9cm H20 in wedge resection group and -14.8cm H20, -4.3cm H20, -8.6cm H20 in ILD group respectively. The range of pleural pressure when forced breathing was wide between -33.1cm H20 and -3.1cm H20 on average. The pleural pressure rose up to 32.0cm H20 (average value) when patients coughed.

Conclusion : We confirmed that the pleural pressure varies widely according to respiration after pulmonary resection. The pleural pressure in lobectomy might be more negative than in wedge resection.
