Uniportal VATS Surgery without Endotracheal Intubation: Single Center Experience

Purpose : Uniportal VATS pulmonary resections have been reported feasible minimally invasive surgical approach. Without endotracheal intubation, nonintubated VATS pulmonary resections draws attention in these days. This study is to describe surgical techniques and show early postoperative outcomes with nonintubated Uniportal VATS pulmonary resections in the single center.
Methods : From January 2017 to July 2017, some 40 consecutive patients received Uniportal VATS pulmonary resections without endotracheal intubation by a single surgeon in our hospital. There were 29 (72.5%) patients were lung cancers undergone pulmonary resections (23 lobectomies and 6 segmentectomies). Other 11 (27.5%) patients were diagnosed with pulmonary metastasis, benign lung diseases or pleural diseases. The multi-level intercostal nerve block using local anesthesia was done in all patients. Intrathoracic vagal blockade was applied in 35 cases (87.5%).
Results : The mean anesthesia time was 166.8 min. The mean operative duration was 125.9 min. The mean postoperative chest tube duration was 3.2 days. The mean hospital stay was 5.8 days. There were three conversions (7.5%) to intubation due to intraoperative hypoxemia. There was one conversion (2.5%) to multiport VATS due to injury on the segmental artery. There were 7 complications (17.5%) among the patients: three prolong air leakage, two chylothorax, one pleural effusion and one pneumonia. Those 7 complications were resolved during the hospital stays. There was no postoperative mortality.
Conclusion : Uniportal VATS surgery without endotracheal intubation appears to be feasible surgical options, and it can be a valid alternative of one-lung ventilated conventional VATS surgery depending on surgeon’s experience.
