Surgical Outcomes of VATS Pulmonary Stapled Segmentectomy

Purpose : This study aimed to evaluate postoperative outcomes of VATS stapled segmentectomy(VATS-SS) in patients with malignant or benign lung diseases.
Methods : From Jul. 2010 to Jun. 2017, sixty-seven patients (34 men; mean age, 59.5±15.0 years) who underwent segmentectomy were consecutively included in this study. 45 patients were treated with segmentectomy alone and the other 22 patients were treated simultaneously with other lung resection. In terms of technical aspect, we investigated the postoperative outcomes of VATS-SS.
Results : 28 primary lung cancers, 22 metastatic lung cancers and 17 benign lung disease were diagnosed. VATS was completed in 57 patients (85.1%), and conversion thoracotomy in 5 patients (7.5%). The most frequent segment removed was the superior segment of LLL or RLL and the lingular segment of LUL. When VATS-SS was performed alone, the mean operation time was 193.0 ± 61.7 min, the length of hospital stay (LOS) was 8.7 ± 4.8 days, the ICU stay was 1 ± 0.2 day and the chest tube drain was removed 5.3 ± 3.1 days after segmentectomy alone. Complications occurred in 14 patients ; 7 persistent air leak (> 7days, 10.4%), 3 pneumonia (4.5%), 2 suspicious remained segmental lung infarction (3.0%) . There is no in-hospital mortality and 30-day mortality.
Conclusion : This study results suggest that VATS-SS is a technically feasible option for malignant or benign lung diseases. The most important point in performing VATS-SS is the special attention to obstruction of segmental vessels and bronchus.
