Follow Up Outcome of Endoscopic Vein Harvest in Coronary Artery Bypass Surgery

Purpose : Endoscopic saphenous vein harvest (EVH) for coronary artery bypass grafting (CABG) has been accepted widely to be effective in reducing wound complications, but there has been a controversy over graft patency and follow-up results. Our purpose is to investigate the early follow-up results of CABG with EVH.

Methods : EVH had been performed on selected patients from April 2013 to February 2017. We studied retrospectively for various perioperative data and follow-up results including patency, repeated revascularization and mortality.

Results : Eighty patients underwent EVH. The mean age was 65.2 year and 64 patients were male. The seventy cases were conventional CABGs, 9 cases were on pump beating heart CABGs and there was one off pump case. The mean procedure time was 9.46 minutes [Range 5 ~ 25 minutes]. Harvested veins were anastomosed on 12 ramus or diagonal branches, 39 branches of left circumflex artery and 50 right coronary branches. There was no wound complication. Cerebral infarction developed in one case. The operative mortality was one case at post operative 30 days due to pneumonia. The mean follow up duration was 25.3 months. Sixty patients had the coronary CT scan at discharge and post operative 6 month. The five of total 71 grafts were occluded [7.04%]. There was no post operative cardiac event and no revascularization case during follow up term.

Conclusion : In selected cases, EVH can be an excellent option for CABG. But, long term follow up and randomized study might be needed.
