Fragility of Red Blood Cells Collected under Different Conditions with a Cell Saver Device: an Animal Study

Purpose : Using a rabbit model, we used a cell saver(CS) device to collect red blood cells(RBCs) from blood lost under different conditions encountered during cardiac surgery and quantified the amount of lethal and sublethal RBC injury.
Methods : Twenty rabbits were divided into two groups, a non-heparinized(NH) group and a heparinized(H) group, according to whether or not they received heparin. Next, blood was directly aspirated from the heart. Each group was further subdivided into a non-gauze(NG) and a gauze(G) group based on whether the aspirated blood was used directly or whether gauze was soaked in the aspirated blood and squeezed. Blood from each of the four groups underwent CS processing. The mechanical fragility index(MFI) and percent hemolysis were measured pre-CS processing and post-CS processing and compared.
Results : In RBCs after CS processing, the MFI and percent hemolysis were significantly increased in both the NH and H groups compared to the values in RBCs collected before CS processing. The H group MFI was significantly higher than that of the NH group in RBCs after CS processing(p=0.002). However, no differences were detected in the percent hemolysis of the H and NH groups post-CS processing(p=0.696). The MFI and percent hemolysis of the NG and G groups did not differ.
Conclusion : This study demonstrates substantial increases in sublethal injuries of RBCs acquired from heparinized blood by CS processing and lethal RBC injuries acquired from non-heparinized blood by CS processing. RBCs collected from a CS device by squeezing blood-saturated gauze did not exhibit additive traumatic effects.
