Electromagnetic Navigation Bronchoscopy-guided localization for Thoracoscopic Resection of Pulmonary Nodules

Purpose : As increasing of computed tomography screening in evaluation of pulmonary diseases, more patients with small pulmonary nodules are being referred to thoracic surgeons for accurate diagnosis. Electromagnetic navigational bronchoscopy (ENB)-guided dye marking wound allow minimally invasive resection for small pulmonary nodules or ground glass nodules. The aim of the present study is to review early experiences of diagnosis or treatment of small pulmonary nodules using ENB.

Methods : We retrospectively reviewed all patients who underwent ENB-guided biopsy or localization (dye marking) followed by minimally invasive resection of pulmonary nodules from Jan 2017 to Jul 2017 at Chungnam National University Hospital. The primary end points were the rate of nodule localization and definitive diagnosis of the nodule.

Results : Twenty-five ENB-guided procedures in 21 patients were performed. Five procedures were ENB-guided biopsy and 19 procedures were ENB-guided localization followed by surgery. In one patient, we abandoned trial of ENB-guided localization. After 19 ENB-guided localizations, 18 wedge resections and 1 segmentectomy were performed using VATS. Sixteen localizations (84%) were performed successfully, but 3 localizations were failed and were identified by palpation. Median size of the nodules was 8mm (range, 3-18mm) and median distance from pleura was 6mm (range 2-31mm). Solid nodule was most common (n=8). Metastasis was the most common pathology (n=10, 53%).

Conclusion : Our initial experiences found that the ENB-guided biopsy or localization followed by minimally invasive resection was feasible, safe, and effective in the diagnosis of small pulmonary lesions.
