Comparison of Oncological Outcomes of Single-port Vesrsus Multi-port Video-assisted Thoracoscopic Lobectomy for Non-small-cell Lung Cancer: A Propensity-matched Analysis

Purpose : To compare oncological outcomes of single-port (SP) versus multi-port (MP) video-assisted thoracoscopic surgery (VATS) lobectomy for non-small-cell lung cancer (NSCLC).
Methods : We started SP VATS lobectomy from May 2013. Overall survival (OS) and disease free survival (DFS) were compared between SP VATS and MP VATS. Between January 2012 and February 2014, 198 consecutive NSCLC patients, who underwent VATS lobectomy by a single surgeon, were enrolled. A propensity-matched analysis, incorporating preoperative variables, was used to compare oncological outcomes between SP and MP VATS lobectomy for NSCLC.
Results : Overall, 53 patients underwent SP VATS lobectomy from May 2013 to February 2014, and 113 patients underwent conventional MP VATS lobectomy from January 2012 to April 2013. Propensity matching produced 53 patients in each group. Median follow-up was 46.9 months (SP) and 57.3 months (MP), respectively. Resected number of lymph nodes (14.5 versus 16.2; p = 0.301) and number of mediastinal lymph nodal stations (2.3 versus 2.5; p = 0.245) were not different (Table 1). OS at 4-year was 90.4% (SP) and 90.6% (MP), respectively (p = 0.972)(Fig 1.). DFS at 4-year was 75.5% (SP) and 71.7% (MP), respectively (p = 0.906) (Figure 2). In the propensity-score matched analysis of SP versus MP VATS lobectomy patients, there was no difference in OS and DFS.
Conclusion : Oncologic outcomes of SP VATS lobectomy for NSCLC are comparable with those of the MP VATS lobectomy.
