Revalidation of the Postneoadjuvant Pathologic TNM Stage of Esophageal Squamous Cell Carcinoma

Purpose : We revalidate 8th AJCC and UICC esophageal cancer staging manual in population of Asian single center patients of advanced esophageal cancer treated by neoadjuvant therapy followed by esophagectomy.

Methods : Between January, 1995 and December, 2014, a total of 442 patients with squamous cell carcinoma in esophagus underwent esophageal resection and reconstruction after neoadjuvant therapy. Preoperatively, 432 (97.7%) patients received chemoradiation therapy, 7 (1.6%) patients had chemotherapy only, and 3 (0.7%) patients underwent radiation therapy only.

Results : Postneoadjuvant pathologic (ypTNM) stage of patients in the study was as follows : Stage I 273 (61.8%), Stage II, 40 (9.0%); Stage IIIa, 63 (14.3%); Stage IIIb, 47 (10.6%); Stage IVa, 14 (3.2%); Stage IVb, 5 (1.1%). Overall survival was 98.1%, 78.2%, 44.1%, and 29.5% at 30 days, 1, 5, and 10 years.. 30 days, 1, 5, and 10 years overall survival by stage were as follows : stage I, 98.1%, 87.5%, 56.4%, and 39.6%, stage II, 97.3%, 78.4%, 26.5%, and 17.7%; stage IIIa, 97.6%, 85.4%, 34.7%, and 17.3%; stage IIIb, 98.3%, 40.7%, 13.3%, and 11.4%;stage IV, 100%, 17.2%, 0, and 0. In comparison with the 8th esophageal cancer staging data, overall survival pattern was similar except reversal of overall survival in stage II and IIIa (p = 0.000).

Conclusion : There was similar pattern between 8th esophageal cancer staging data and ours. However, stage II was worse overall survival than stage IIIa. Further study is warranted to discover these findings and to optimize esophageal cancer staging for Asian patients with esophageal cancer.
