Clinical Impact of Atrial Activity after Maze Procedure for the Patients with Atrial Fibrillation

Purpose : Restoration of normal sinus rhythm is the primary goal of maze procedure for atrial fibrillation. Also, restoration of atrial contraction is another important purpose of this procedure, to increase the cardiac output and to decrease the incidence of thromboembolism. This study evaluated the impact of restored atrial contractility on clinical outcome and echocardiographic parameters after maze procedure.

Methods : Between January 2004 and December 2016, 239 patients underwent maze procedure. Among them, 140 patients who had long-term (more than 1 year) data were finally enrolled. Electrocardiography and echocardiographic parameters at immediate postoperative, postoperative 1-year and last follow-up date were retrospectively analyzed. The patients were divided into group A (presence of A-wave, N=55) and group B (absence of A-wave, N=85).

Results : There was no early mortality in both groups. The rate of overall mortality was lower in Group A (n=1, 1.8 % : n=18, 21.2 %, p= 0.001). Radiofrequency ablation showed lower restoration rate compared with cryoablation (30.2 % vs 59.1 %, p=0.001). The incidence of morbidities was significantly lower in Group A (n=16, 29.1 % : n=52, 61.2 %, p<0.001). There was no difference in LVEF (57.2±10.2 % vs 54.7±11.8 %, p=0.197), LA diameter (46.6±7.0 mm vs 49.9±10.7 mm, p=0.119), and LVEDD (48.3±6.6 mm vs 50.2±7.2 mm, p=0.118) between group A and B, respectively.

Conclusion : Restoration of atrial activity after maze procedure was associated with improved clinical outcomes. But, there was no difference in change of echocardiographic parameters.
