Clinical Result of Arteriovenous Fistulae using Autologous Vessel in end Stage Renal Disease Patients on Hemodialysis

Purpose : For hemodialysis patients with end stage renal disease(ESRD), it is important to construct efficient vascular assess with superior patency rate. This study was designed to identify factors that influence the construction of efficient arteriovenous fistula(AVF) using autologous vessel and to evaluate the necessity of ultrasonography as a preoperative tool to construct an AVF.

Methods : From January 2009 to April 2016, among 634 cases who had AVF due to ESRD at our institute, retrospective analysis was carried out on 250 patients who had AVF using autologous vessel.

Results : 1, 3 and 5 year patency rates for all subjects were 87.6%, 85.6%, and 84.4%. Among them, the group who had preoperative evaluation of their vessels via ultrasonography, had better patency rate compared to the group who had not. Patency rate in group under 65, or anastomotic vein diameter 3mm or more had superior patency rate. 1 year patency rate and the diameter of anastomotic vein showed positive correlation.

Conclusion : Ultrasonography is highly recommended for AVF and efforts should be made to increase patency rate in patients over 65. Superior clinical result can be expected when AVF is made using autologous vessel with diameter of anastomotic vein longer than 3mm.
