Surgical Treatment of Huge Empyema which Makes Total Compression of Ipsilateral Lung Parenchyma and Induces Shifting of Mediastinum Resulting in Cardiopulmonary Dysfunction

Purpose : A 57-year-old male patient was admitted to the hospital presenting with severe dyspnea. Chest computed tomography showed a huge heterogenous mass suggesting empyema with total compression of right lung parenchyma and shifting of mediastinum contra-laterally resulting in severe cardiopulmonary dysfunction. We performed empyemectomy and pulmonary decortication with interposition of latissimus dorsi muscle without right pneumonectomy. The patient discharged on the 21st post-operative day without any significant complications and showed outstanding performance with NYHA grade I~II status at 8 months after surgical intervention.
