Early Clinical Outcomes of Tricuspid Valve Repair with Tri-Ad Annuloplasty Ring; Is it better or Worse than MC3 Ring?

Purpose : We evaluated the early clinical outcomes of tricuspid valve annuloplasty with Tri-Ad annuloplasty ring for functional tricuspid regurgitation (TR).
Methods : From Jan 2015 to Mar 2017, 36 patients underwent TAP with Tri-Ad for functional TR. To evaluate early clinical outcomes of TAP with Tri-Ad, we compared it to MC3 annuloplasty ring's by propensity score-matched analysis (n=34 in each group). The follow-up duration was 11.0 ± 7.07 months.
Results : There was 1 (2.8%) operative mortality and no late mortality. Postoperative complications occurred in 15 (41%) patients, including AKI in 6 (16%) patients, bleeding reoperation in 4 (11%) patients, and low cardiac output syndrome in 4 (11%). There was no ring-related complication, such as atrioventricular block or ring dehiscence. TR grade (from 2.03 ± 1.06 to 1.18 ± 0.92, p < 0.01) and systolic pulmonary artery pressure (from 43.53 ± 13.84 mmHg to 38.00 ± 9.72 mmHg, p = 0.03) decreased significantly. There was no residual severe TR, but moderate TR was observed in 3 patients who had severe TR preoperatively. Preoperative severe TR was associated with residual TR ≥ moderate in univariate analysis (p < 0.01). In propensity score-matched analysis with MC3 group, there was no significant difference in early clinical outcomes.
Conclusion : TAP with Tri-Ad corrects functional TR effectively and provides good early clinical and echocardiographic results without ring-related complications. However, the preoperative severe TR was associated with residual TR ≥ moderate in the immediate postoperative period. The long-term follow-up study is mandatory to confirm the stability of this procedure.
